ASABA/Nigeria: Delta State Ministry of Health has requested all operators of public and private Health Facilities, primary and secondary, tertiary, Traditional Birth Attendants (TBA) in the State to submit Health Data monthly to the Health Management Information System (HMIS) officer in the Local Government Areas in the State, with effect from 1st January, 2019.
A statement signed by the Director of information, Mr Paul Osahor stated that forms to be used for the submission of the health data include Health Facility Level Routine Monthly Notification Form–Integrated; Disease Surveillance Response; Maternal and Child Welfare Clinic Form; National Health Management Information System (NHMIS) Health Facility Monthly; Generic Cancer Abstract Form if there is a case of cancer and Tool for Community Audit of Maternal Deaths if there is a case of maternal death.
The statement further indicated that operator of the health facility were to be responsible for making copies of relevant forms, completing them and submitting to the HMIS Officer in the Local Government Area where the health facility were located by the second Monday of the following month.
Operators of health facilities were also advised that a copy of all the data submitted be kept in the health facility for validation and record purposes. Other forms on health data should continue to be submitted using the usual channels.
It stated that records of those submitting data monthly would be kept and the names of those who default would be compiled for appropriate sanctions and officers in charge of all health facilities should avail themselves of the training course on health data management being organised at the Federal Medical Centre, Asaba on 14th December 2018 to receive appropriate training on how to generate, collate and submit all relevant data as submission of inaccurate data would not be tolerated.
